Options for having all form fields or select form fields similar to InfoPath Starter Form on Design Surface

Give user the options whether the form fields should be automatically be on the Design Surface or the users can drag and drop the form fields/answers themselves.

The main reason is that most of the time, we only include selected coded answers for certain questions when the form field is a multi-select.
