Repeat concept name when adding a new obs from 'Most Recent Observation'

The 'Most Recent Observation' UI is a bit confusing when adding a new date datatype (see attached screen shot). It is not immediately clear which field is the obs value, and which field is the obs datetime.

It might help to repeat the concept name in the yellow box, ie:

New RETURN VISIT DATE [value_datetime] on [obs_datetime] (dd/mm/yyyy) [Save] [Cancel]

This could probably be done for all concept datatypes.

If possible, this should be implemented in 1.5 but backported to 1.4.
