Should not allow saving a Person Attribute Type without a Format

Problem: The Person Attribute Type without a Format specified is allowed to save.

Steps to Reproduce:
1. Use the OpenMRS 1.11x version code and go the below link (with admin rights):
http://localhost:8080/openmrs/admin/person/personAttributeType.form

2. Add the other values but don't select the Format Type (see attached personattributetype1.png)

3. Try to create a person with the link:
http://localhost:8080/openmrs/admin/person/addPerson.htm?viewType=edit
