improve patient search support for multiple names

      Description

      When you search for a patient by name, and that patient has more than one name that matches the query, then multiple entries come up for that patient on the search page (though only the preferred name is displayed).

      E.g., if you have a patient with the following names:
      Bob Marley
      Robert Marley
      Robert Marley III (preferred)

      And you search for Marley, then the patient search page will give the following result:

      Robert Marley III
      Robert Marley III
      Robert Marley III

      It should just return

      Robert Marley III

      Say you have a patient with names

      Ephraim bin Fischal (my hebrew name)
      Chase Garrett Yarbrough (my full name) (preferred)

      A search for "Ephraim" would yield

      Chase Garrett Yarbrough
      Chase Garrett Yarbrough
      Chase Garrett Yarbrough

      Should be more like:

      Search: "Chase"
      1. Chase Garrett Yarbrough

      Search: "Garret"
      1. Chase Garret Yarbrough

      Search: "Ephraim"
      1. Ephraim bin Fischal (Chase Garret Yarbrough)
