Error message produced when problem arises exiting a patient from care is not very helpful

      Description

      We recently had a situation on the Rwanda Version: 1.6.2 dev Build 12911 OpenMRS. A data entry clerk was trying to exit a patient from care using a date that was prior to the date the patient was enrolled in the program. Quite correctly OpenMRS prevented this from happening, however the error message displayed to the user was a pop-up that read "Internal error while trying to process patient death - unable to proceed". The stacktrace for the exception was also only being output at debug level, meaning that is was difficult to identify the cause of the problem.

      I have created a patch for trunk which appends the error message from the root exception to the error message being shown to the user (as in this case the exception message was human readable and useful) along with changing the logging level to warn, to ensure this kind of exception will always get logged (as it is always likely to need to be investigated).

      Not a big enough issue to bother with backporting, but is probably worth getting sorted out in trunk
