Ability to "Add New Person" when adding a relationship on Create a New Patient Page

When adding a relationship under the patient dashboard, if the person can't be found, there is a button to "Add New Person". This button should also be available when adding a relationship on the "Create a New Patient Page."

This is related to the global property, newPatientForm.relationships.

This is an enhancement to Ticket TRAC-1050.
