Patient Image Module 0.4 doesn't work; no window shown to take a patient picture

    Description

      The Patient Image Module version 0.4 doesn't properly show the window to take a patient's picture. See the attached screenshot. This happens with Flash 11.4 on FireFox 15.0, 15.0.1, 16.0 and IE 9. Version 0.3.3 works properly in the same environment (with the exception that the photo saved is completely black/no image at all).
