Add "providers" attribute to <encounterProviderAndRole> tag

This attribute will work similar to the <encounterProvider persons="username1,username2"/> tag, but will accept a comma separated list of provider ID's or UUID's. For example, <encounterProviderAndRole providers="33,4a72caf0-3840-102d-994e-001517385830"/>.

Also, consider allowing Provider Identifiers in the list which may be more user friendly.
