Autocomplete observations can't be deleted once selected

      Description

      Once an autocomplete value has been selected, it's not possible to delete it from the form.

      For example:

      1. I have a basic autocomplete for a Drug on my form
      2. The user selects "Ibuprofen" using autocomplete
      3. The user decides they don't want to give the patient Ibuprofen and completely clear the text box
      4. User enters the form
      5. Editing/Viewing the form reveals that, Ibuprofen has still been submitted with the form! The user has no idea that this drug was entered.

      Additionally, if a user is editing a previously saved form and clears an autocomplete textarea then and saves the form, the original value will not actually be cleared. In both cases it IS possible to change the drug given. The only way to clear the autocomplete is to delete the entire form.

      This is very serious issue because erroneous data can be entered on a patient without the user knowing.
