Warn provider starting consultation if they already have an appointment that is in-consultation

When the user starts a consultation either from:

• The appointment management form (with the start consultation button or the popup after clicking the patient's name)
• The patient dashboard "start consultation" button

the module should check to see if the provider listed for the appointment has another appointment with status of "in-consultation." If YES, the user should be shown a popup that says "This provider already has active consultation(s). Are you sure you want to start another?" with options: Continue & Cancel
